HERE ARE THE Mirchandani VISIT NOTES: 

He sees the issue as L4 Illiad Crest, L5 SI joints.  

Sees L3/4 as Arthritis shown in the MRI.  And may have more than one issue!  

An IV Epidural to work on L3/4, perhaps.

Dr. Mirchandani's Overall plan:

1st will try steroid injections

2nd. Nerve block Epidural 3 months (2 pain injects of lidocaine to test and must have 88% pain relief for insurance to pay for Epidural) or Radio Frequency Aplasian treatment.  

Dr. finds it weird that MRI report doesn't mention Arthritis in lower back

Ordered Nortirptoline 10/20 mg

Will set to go on Bilateral Lumbar Transforaminal Epidural Steroid Injection w/o sedation of Corticosteroid into epidural space.

April 19 2020 - One of the best days

 - Tim and Mary came to visit and Nathan had lower pain, a great sense of humor and it was nice not to be as focused on how hard days were.  We loved this visit.  All of us!

April 23 -Began Gabapintin - What to do to help get off of the overdosing  on Aleve!

April 29 Saw Dr. Mirchandani, DO,PSV - 223 lbs   

April 30  -First Steroid injection in L3 by Dr. Mirchandani
